If you get enough brain damage, you can hallucinate runners and lurkers in your peripheral vision, right at the edge of the darkness.
Would be funny to see brain damaged people going crazy and shooting into the shadows.

I wouldn’t be opposed to minor to moderate hallucination-like effects (The same as neuro-gas, just less obnoxious.) - brain damage is pretty hard to self-diagnose compared to the rest of the organ damages, so more prominent effects would kill two birds with one stone.
